We provide personal care and support programs to help members with individual health challenges and situations, from emotional and mental health to pregnancy to cancer, chronic and complex conditions.

Coordinated care with you at heart.

Composed of nurses, social workers, diabetes health educators and pharmacists, our personalized care teams make sure members like you, with complex or chronic conditions, get the care you need and deserve. The program includes the following services:

  • An individual care manager to coordinate and communicate with your doctor, hospital and specialists, answer questions, monitor your condition, simplify how the system works for you.
  • Help in finding in-network providers such as local doctors, specialists, mental health providers, clinics, and diagnostic testing and imaging locations. 
  • Help with appointments, prescriptions, scheduling hospital visits, follow-ups, routine screening reminders and referrals, therapy – even claims.
  • Regular check-ins with your care manager by phone, virtual chat and home visits.
  • Connecting you with convenient care options such as Anytime-MD, house calls via DispatchHealth, MinuteClinic®, and HealthHUBTM, plus referrals to community resources.
  • Education on chronic disease and condition management and regular assistance via automated text-message reminders.

If your doctor recommends you, we’ll reach out directly. So, if you receive a call or text from Texas Health Aetna, please take it: it’s our Care Team calling to see how they can help.

They can also tell you more about specific programs relevant to you and your family such as Case Management, Disease Management, Compassionate Care, Behavioral Health, Healthy Lifestyle Coaching (1-866-213-0153) and the 24-hour nurse line free to all members where you can speak to a registered nurse on a range of health topics (1-800-556-1555).

Expect better: our Maternity Program.

Run by deeply experienced nurses, our Maternity Program ensures personal support through pregnancy, birth and beyond. It’s free for all Texas Health Aetna members who are mothers-to-be, and offers the following services:

  • Educational materials on prenatal care, labor, and delivery and newborn care
  • Fertility Advocate for those trying to conceive
  • Genetic screening and counseling
  • Personalized nurse support
  • Understanding signs and symptoms of preterm labor
  • Individual counseling to stop smoking or other harmful habits
  • Managing the risk factors associated with your pregnancy
  • Helping introduce and maintain healthy lifestyle changes 
  • Advice on your child’s immunization and vaccination schedule
  • Guidance on harmless/harmful prescriptions
  • Weekly health and safety tips through text4baby: text BABY (BEBE en espanol) to 511411.
  • Access to Maternity Support Center: a digital hub with plan-specific info on costs and tests, educational videos and the latest expert clinical content.

Concentrated support and care for cancer patients.

Whether you’ve just been diagnosed, are in the middle of treatment, or caring for a loved one, we’re there to help you through it all, letting you know exactly what to expect at each phase, and how to cope and look forward.

Complementing the care and guidance from your doctors and specialists is our Digital Cancer Support Center, a hub for resources, tips, information and tools on everything from prevention and screening to understanding your diagnosis, navigating the treatment process and afterwards. Simply use your member log in and access the Cancer Support Center under the Find Care menu.

Get the confidence and coaching to cope.

Whatever happens in life, we’re here to help. Our AbleTo program provides members with confidential one-on-one therapy and coaching. Over eight weeks, and two weekly phone/video appointments with both a therapist and a behavior coach, you’ll establish the mental and physical strategies to manage your emotional challenges, and get your life and health back on track.

AbleTo can help with issues arising from:

  • Infertility
  • Cancer recovery
  • Heart issues
  • Recent major surgery
  • Diabetes
  • Digestive health issues
  • Chronic pain management
  • Breathing problems
  • Alcohol or substance abuse
  • Depression, anxiety or panic
  • Postpartum depression
